Dexmedetomidine Dosing for Cesarean Section

Efficacy and Safety

  • The PROSPECT 2021 guidelines for cesarean section evaluated multiple studies on intrathecal dexmedetomidine and concluded that it improves postoperative analgesia when compared to bupivacaine or ropivacaine alone, with a recommended dose of 3-5 μg as an adjuvant to local anesthetic 1, 2, 3.
  • The American Society of Anesthesiologists recommends dexmedetomidine intrathecal as an adjuvant to improve analgesia, with a dose of 3 μg providing effective analgesia without significantly prolonging motor block 4.

Combination Therapy

  • The combination of dexmedetomidine intrathecal with magnesium sulfate intrathecal or morphine intrathecal improves analgesia with longer duration, according to the PROSPECT guidelines 1, 2, 4.
  • It is recommended to avoid combining fentanyl intrathecal with morphine intrathecal, as it may induce acute opioid tolerance and result in increased opioid consumption 1, 2, 4.

Multimodal Analgesia

  • The PROSPECT guidelines recommend multimodal analgesia, including paracetamol 1000 mg every 6 hours, NSAIDs (ibuprofen 400-600 mg every 6-8 hours or diclofenac), and dexamethasone IV 8 mg single dose after delivery 5.

Important Considerations

  • The European Society of Anesthesiology recommends not omitting basic analgesics (paracetamol/NSAIDs) when using dexmedetomidine intrathecal, as this increases opioid requirements 6.
