Paliperidone Palmitate Initiation and Management
Patient Selection and Initiation
- The American College of Psychiatry recommends a brief oral risperidone or paliperidone trial to assess tolerability before committing to a long-acting formulation in patients with first antipsychotic exposure ever or uncertain tolerability profile 1
- Patient preference should be considered when deciding to start with oral medication, as shared decision-making suggests starting with oral medication may be beneficial 2, 1
Treatment Outcomes and Monitoring
- Significant symptom improvement compared to placebo is typically observed after the first loading dose by Day 8, with continued improvement and dose-dependent response after the Day 8 injection, and full assessment of treatment response should occur by Week 4 3, 4, 1
- Orthostatic hypotension and prolactin-related symptoms, such as sexual dysfunction, galactorrhea, and menstrual irregularities, should be monitored 5, 1
- Weight and metabolic parameters, as well as psychotic symptom severity using standardized scales, should be monitored 1, 5, 1
Special Considerations
- The British Journal of Psychiatry suggests that treatment should not be delayed waiting for an oral trial if the patient has documented non-adherence or prefers injectable medication 2
- The American Journal of Psychiatry recommends allowing adequate time at therapeutic doses before switching or augmenting treatment, and not assuming treatment failure before 4 weeks 3, 4, 1
