Cardiac Hypertrophy Assessment and Management

Introduction to Cardiac Hypertrophy Assessment

  • The European Society of Cardiology and European Association of Cardiovascular Imaging (EACVI) recommend assessing and grading left ventricular hypertrophy (LVH) using left ventricular mass (LVM) indexed to body surface area (BSA), with specific thresholds for mild, moderate, and severe LVH based on gender 1, 2

LVH Measurement and Calculation

  • The American Society of Echocardiography (ASE) recommends calculating LV mass using the formula: LV mass (g) = 0.8 × {1.04 × [(LVIDd + PWTd + SWTd)³ - (LVIDd)³]} + 0.6, where LVIDd = LV internal diameter in diastole, PWTd = posterior wall thickness in diastole, SWTd = septal wall thickness in diastole 1
  • Measurements should be taken at end-diastole, using the inner-edge-to-inner-edge method for linear dimensions, and positioning the cursor perpendicular to the long axis of the ventricle 1, 2

LVH Classification and Thresholds

  • For women, normal LV mass index is 43-95 g/m², mild LVH is 96-108 g/m², moderate LVH is 109-121 g/m², and severe LVH is ≥122 g/m² 1, 2
  • For men, normal LV mass index is 49-115 g/m², mild LVH is 116-131 g/m², moderate LVH is 132-148 g/m², and severe LVH is ≥149 g/m² 1, 2
  • For overweight/obese patients, consider height-based indexing, with normal LV mass/height²·⁷ (g/m²·⁷) being ≤44 g/m²·⁷ (women) and ≤48 g/m²·⁷ (men) 1

Relative Wall Thickness (RWT) Assessment

  • RWT is calculated as (2 × PWTd) / LVIDd, with normal RWT being 0.22-0.42 (women) and 0.24-0.42 (men) 1, 2

Geometric Patterns and Cardiovascular Risk

  • Concentric hypertrophy carries the highest cardiovascular risk among the geometric patterns, which include normal geometry, concentric remodeling, eccentric hypertrophy, and concentric hypertrophy 1
  • Septal morphology and wall hypertrophy localization should be documented in cases of asymmetric hypertrophy 3
  • The following table summarizes the geometric patterns and their corresponding RWT values:
Geometric Pattern RWT Value
Normal Geometry 0.22-0.42 (women), 0.24-0.42 (men)
Concentric Remodeling >0.42
Eccentric Hypertrophy <0.42
Concentric Hypertrophy >0.42

Common Pitfalls in LVH Assessment

  • Relying solely on wall thickness measurements rather than calculated LV mass is a common pitfall, as is not indexing LV mass to body size (BSA or height) 1
  • Using BSA indexing in obese patients, instead of height-based indexing, can lead to inaccurate assessments 1
  • Failing to assess RWT and geometric pattern can result in incomplete prognostic information 1, 3
  • The ASE and EACVI guidelines strongly recommend using indexed LV mass rather than wall thickness alone for LVH diagnosis and severity assessment, as it provides superior prognostic information and more accurately reflects true LV hypertrophy 1, 2
  • 3D echocardiography may be more accurate for asymmetric or localized hypertrophy and avoids geometric assumptions in abnormally shaped ventricles 2
