Contrast Media Use and Renal Safety in Imaging

Contrast Volume and Type

  • The American College of Radiology recommends using the lowest diagnostic dose of iodinated contrast necessary for the study to limit renal exposure. 1
  • The American College of Radiology advises preferring low‑osmolar or iso‑osmolar iodinated contrast agents and avoiding high‑osmolar agents whenever possible. 1

Alternative Imaging Strategies

  • When the clinical question can be answered without iodinated contrast, the American College of Radiology suggests using non‑contrast CT or ultrasound as alternative imaging modalities. 1

Management of Nephrotoxic Medications

  • For patients with an estimated glomerular filtration rate (eGFR) < 60 mL/min/1.73 m², metformin therapy should be reviewed and discussed with the ordering clinician before contrast administration. (Radiology) 2

Patients on Maintenance Dialysis

  • The American College of Radiology states that individuals receiving maintenance dialysis may undergo contrast‑enhanced CT without additional prophylactic measures because they have no residual renal function to protect. 1
  • Radiology reports that dialysis schedules do not need to be altered or newly initiated solely because of contrast administration. 2
