Rituximab Administration and Efficacy

Mechanism and Depletion

  • B-cell depletion is sustained for 6-12 months in most patients, with recovery beginning at approximately 6 months and median B-cell levels returning to normal by 12 months, as reported by the British Journal of Dermatology and Praxis Medical Insights 1, 2

Administration and Monitoring

  • Obtain immunoglobulin levels before initiating therapy, as recommended by Praxis Medical Insights 3
  • Screen for hepatitis B and C antibodies, including occult hepatitis B infection, as recommended by Praxis Medical Insights 3
  • Monitor complete blood count at 2-4 month intervals during treatment for cytopenias, as recommended by Praxis Medical Insights 3

Dosing Regimens and Efficacy

  • The standard schedule for non-Hodgkin's lymphoma is 375 mg/m² IV once weekly for 4 weeks, with overall response rates of 30-60% and duration of response of 8-11 months, as reported by Blood 4
  • For rheumatoid arthritis, the dosing regimen is 1000 mg IV on day 0, repeated on day 15, with maintenance dosing of 1000 mg IV every 24 weeks based on clinical response, as recommended by Praxis Medical Insights 3
  • For inflammatory myositis, the primary regimen is 1000 mg IV on day 0, repeated on day 15, with an alternative regimen of 375 mg/m² IV once weekly for 4 consecutive weeks, as recommended by Praxis Medical Insights 3
  • For bullous pemphigoid, the dosing regimen is 375 mg/m² IV once weekly for 4 weeks, with repeat infusions in some cases, as reported by the British Journal of Dermatology 2

Safety and Special Considerations

  • Serious adverse events, including fatal infections, may occur, particularly with prolonged B-cell depletion or hypogammaglobulinemia, as reported by the British Journal of Dermatology 2
  • Progressive multifocal leukoencephalopathy is a rare but fatal complication requiring vigilance, as recommended by Praxis Medical Insights 3
  • Elderly patients may receive lower dose intensity of rituximab and concomitant chemotherapy, and may be at higher risk for serious adverse reactions, including infections, neutropenia, and pancytopenia 4
