Intrathecal Morphine-Bupivacaine Pump for Chronic Non-Malignant Pain
Mandatory Prerequisites Before Pump Implantation
- The American College of Physicians recommends that patients with chronic non-malignant pain must have failed conservative management, including systemic opioids and adjuvant analgesia, before proceeding with permanent pump implantation, with a life expectancy of more than 6 months 1, 2, 3
- Patients must have undergone a successful trial period, demonstrating ≥50% pain reduction, and have completed a mandatory psychological evaluation to assess suitability for permanent pump implantation 1, 2, 3
Contraindications
- The American Society of Regional Anesthesia and Pain Medicine states that intrathecal pump therapy is contraindicated in patients with active infections, coagulopathy or bleeding disorders, and very short life expectancy (<6 months) 2, 3
Dosing Considerations
- The European Society of Regional Anaesthesia and Pain Therapy recommends that typical intrathecal dosing regimens for chronic non-malignant pain include morphine initial doses of 1.1-1.2 mg/day and bupivacaine concentrations of 0.125%-0.25%, with doses ranging from 2.5-12.6 mg/day 2, 3
- The intrathecal route requires only 1/300th of the oral morphine dose and 1/24th of the epidural dose for equivalent analgesia, according to the American Pain Society 2, 3
