IVIG in BK Virus Associated Nephropathy

Introduction to BKVAN Management

  • The American Society of Transplantation recommends reduction of immunosuppression as the first-line management for BKVAN, as this is the only proven therapy according to current guidelines 1

Monitoring and Treatment Approach

  • Serum creatinine and renal function should be monitored, as recommended by the American Journal of Transplantation and Praxis Medical Insights 1, 2
  • Repeat biopsy should be considered if renal function deteriorates despite viral load reduction, as suggested by Praxis Medical Insights 2
  • Diagnosis of BKVAN should be made via biopsy with confirmatory immunohistochemistry or in-situ hybridization, as recommended by the American Journal of Transplantation and Praxis Medical Insights 1, 2

Important Considerations

  • Acute rejection risk increases substantially after immunosuppression reduction, occurring in 59-79% of patients, and may require pulse corticosteroids and careful differentiation from BKVAN progression, as noted by the American Journal of Transplantation and Praxis Medical Insights 1, 2
