Depakote Dosage and Administration

Introduction to Depakote Dosage

  • The American Academy of Family Physicians recommends an initial dose of 125 mg twice daily for mood stabilization in bipolar disorder, with a total daily dose of 250 mg, and titration upward to achieve therapeutic blood levels of 40-90 mcg/mL 1
  • The American Academy of Child and Adolescent Psychiatry suggests titrating gradually to therapeutic blood level (40-90 mcg/mL) for mood stabilization in bipolar disorder 1, 2

Status Epilepticus Treatment

  • For acute seizure control in status epilepticus, a loading dose of 20-30 mg/kg IV is recommended, with an infusion rate of up to 6-10 mg/kg/min, which produces efficacy in 63-88% of patients 3, 4, 5, 6, 1
  • This approach is safe even with concurrent psychotropic medications and is superior to phenytoin for convulsive status epilepticus (66% vs 42% efficacy) with fewer adverse effects 3, 4, 5

Monitoring and Maintenance

  • The American Academy of Child and Adolescent Psychiatry recommends checking valproate levels to confirm therapeutic range (40-90 mcg/mL for mood stabilization; 50-100 mcg/mL for seizures) and monitoring liver enzymes, complete blood count (especially platelets), and coagulation parameters as indicated 1, 2
  • Once stable, the American Academy of Child and Adolescent Psychiatry suggests checking levels every 3-6 months 2
