Rivaroxaban Dosing for Stroke Patients with Impaired Renal Function

Dosing Algorithm Based on Renal Function

  • For patients with normal or mild renal impairment (CrCl ≥50 mL/min), the American Heart Association recommends rivaroxaban dosing at 20 mg once daily 2
  • For patients with moderate renal impairment (CrCl 15-50 mL/min), the American College of Cardiology recommends rivaroxaban dosing at 15 mg once daily 1, 2

Monitoring and Safety Considerations

  • Renal function should be assessed at baseline and periodically during treatment, especially in elderly patients or those with known renal impairment, as recommended by the European Society of Cardiology 4
  • Baseline and subsequent regular assessment of renal function (by CrCl) is recommended in patients following initiation of rivaroxaban, which should be done annually but more frequently (2-3 times per year) in those with moderate renal impairment, according to the European Heart Journal 5

Important Warnings and Precautions

  • Rivaroxaban should be avoided in patients with severe renal impairment (CrCl <15 mL/min) due to increased risk of bleeding, as warned by the American Academy of Family Physicians 3
