Risperidone Contraindication in Parkinson's Disease

Alternative Antipsychotics

  • The American Academy of Child and Adolescent Psychiatry recommends quetiapine, clozapine, or pimavanserin as alternative antipsychotics for patients with Parkinson's disease, with quetiapine being well-tolerated and having minimal impact on motor function 1
  • Pimavanserin, a selective 5-HT2A inverse agonist, is specifically approved for Parkinson's disease psychosis, as recommended by the American Geriatrics Society 2
  • The American Academy of Family Physicians suggests starting with low doses and titrating slowly, with specific dosing considerations for quetiapine and clozapine, such as an initial dose of 12.5 mg twice daily for quetiapine and starting at very low doses for clozapine 3

Mechanism of Harm

  • The American Academy of Child and Adolescent Psychiatry notes that among atypical antipsychotics, risperidone appears to be the most likely to produce extrapyramidal side effects due to its higher affinity for D2 dopamine receptors compared to quetiapine and clozapine 4, 1
