Dosing of Cyclophosphamide in Patients with Impaired Renal Function

Introduction to Cyclophosphamide Dosing

  • The American College of Rheumatology recommends that the dose of cyclophosphamide should be reduced according to age and serum creatinine level, using regimens of 7.5-15 mg/kg per intravenous pulse (maximum 1500 mg) or 1-2 mg/kg/day orally, with specific adjustments to avoid hematologic and infectious toxicity 1, 2.

Dosage Adjustment for Intravenous Pulses

  • For ANCA-associated vasculitis and severe kidney disease, recent guidelines establish specific reductions based on renal function and age 1, 2.

Table of Dose Adjustment for IV Pulses

Age Creatinine Level Dose
<60 years <300 μmol/L 15 mg/kg/pulse [1]
<60 years 300-500 μmol/L 12.5 mg/kg/pulse [1]
60-70 years <300 μmol/L 12.5 mg/kg/pulse [1]
60-70 years 300-500 μmol/L 10 mg/kg/pulse [1]
>70 years <300 μmol/L 10 mg/kg/pulse [1]
>70 years 300-500 μmol/L 7.5 mg/kg/pulse [1]

Frequency of Administration

  • The recommended IV pulse regimen is 15 mg/kg (maximum 1500 mg) initially every 2 weeks, then reduced to every 3 weeks, continuing for 3-6 months for induction of remission 2, 3.

Oral Daily Dosage

  • For continuous oral therapy, doses should be adjusted more conservatively 3, 4.
  • Standard dose: 1-3 mg/kg/day (maximum 200 mg/day) 4, 3.

Critical Safety Considerations

Mandatory Bladder Protection

  • MESNA (2-mercaptoethanesulfonate sodium) is mandatory with IV pulses to prevent hemorrhagic cystitis, which occurs in 6% of patients without protection 3.

Antimicrobial Prophylaxis

  • Trimethoprim/sulfamethoxazole 800/160 mg on alternate days or 400/80 mg daily is mandatory for Pneumocystis jirovecii prophylaxis during the entire course of cyclophosphamide or for 6 months after rituximab 2, 3.

Cumulative Dose Limits

  • The total cumulative dose should be strictly limited due to the risk of secondary malignancies 3, 5.
  • General limit: Do not exceed 36 g of total cumulative dose (preferably <25 g) 5.
  • Patients who wish to conceive: Maximum 10 g cumulative 5.

Disease-Specific Context

ANCA-Associated Vasculitis with Creatinine >4 mg/dL

  • In severe kidney disease (serum creatinine >4 mg/dL or >354 μmol/L), cyclophosphamide remains the preferred agent over rituximab due to limited data on the latter in this population 6, 2.

Required Monitoring

  • During cyclophosphamide treatment, the following monitoring is required:
