Duration of Moxifloxacin for Acute Appendicitis

Treatment Duration Based on Clinical Context

  • The World Journal of Emergency Surgery recommends moxifloxacin 400 mg orally once daily for 7 days total for uncomplicated acute appendicitis treated non-operatively 1, 2, 3
  • The WSES Jerusalem Guidelines specify that most randomized controlled trials included a minimum of 48 hours of intravenous antibiotics followed by oral antibiotics for a total duration of 7-10 days for uncomplicated acute appendicitis 1, 2, 3
  • For patients with beta-lactam allergy requiring moxifloxacin specifically, the regimen is moxifloxacin 400 mg once every 24 hours as part of the 7-10 day course for uncomplicated acute appendicitis 1, 2, 3
  • For patients undergoing appendectomy for uncomplicated appendicitis, a single preoperative dose of moxifloxacin is recommended, with no postoperative antibiotics 4, 5, 6, 7, 8, 9, 10
  • For complicated appendicitis with adequate source control, moxifloxacin should be administered for 3-5 days maximum postoperatively 4, 5, 6, 7, 8, 9, 10
  • Non-operative management with antibiotics is only appropriate for CT-confirmed uncomplicated appendicitis without appendicolith, as recommended by the World Journal of Emergency Surgery 1, 2, 3
  • For patients ≥40 years old treated non-operatively, colonoscopy and interval CT scan are recommended due to the 3-17% incidence of appendiceal neoplasms in this age group 6, 9

Duration of Moxifloxacin for Acute Appendicitis

Non-Operative Management

  • The World Journal of Emergency Surgery recommends that patients with CT-confirmed uncomplicated appendicitis without appendicolith who choose non-operative management should be aware of the 23-39% recurrence risk over 5 years 11
  • The absence of appendicolith on imaging is a critical prerequisite for non-operative management, as appendicolith presence increases failure rates to 47-60% 11
  • Recurrent appendicitis after initial successful antibiotic treatment occurs in approximately 11-14% at 1 year 11

Alternative Regimens

  • The World Journal of Emergency Surgery notes that most randomized trials used minimum 48 hours intravenous antibiotics followed by oral antibiotics for a total duration of 7-10 days 11
