Anticoagulation in Hemodialysis Patients

Prophylactic Subcutaneous Heparin in Dialysis Patients

  • All hospitalized dialysis patients without contraindications should receive pharmacologic VTE prophylaxis, with LMWH preferred over UFH when creatinine clearance permits, as recommended by the American College of Chest Physicians 2, 1
  • For severe renal impairment (CrCl <30 mL/min), UFH is actually the preferred agent over LMWH due to hepatic metabolism rather than renal clearance, making it safer in this population, according to the National Comprehensive Cancer Network 3, 1, 4, 5
  • Standard prophylactic dosing is UFH 5,000 units subcutaneously either twice or three times daily, with three-times-daily dosing showing superior efficacy in surgical patients but higher bleeding risk in medical patients, as stated by the Journal of the National Comprehensive Cancer Network 3, 1
  • UFH prophylaxis has demonstrated mortality reduction (7.8% vs 10.9%) and decreased DVT incidence (4% vs 26%) in critically ill patients, including those with renal failure, as reported by Praxis Medical Insights 1
  • The primary contraindication is heparin-induced thrombocytopenia (HIT), not the dialysis procedure itself, as noted by the Journal of the National Comprehensive Cancer Network and Anaesthesia 3, 6, 4

Special Considerations and Pitfalls

  • Active HIT or history of HIT requires alternative anticoagulation (argatroban, danaparoid, or citrate), as recommended by Anaesthesia and Praxis Medical Insights 6, 4
  • For VTE prophylaxis in hospitalized dialysis patients, the American College of Chest Physicians recommends checking for HIT history or active HIT, and if present, using fondaparinux or danaparoid, and if CrCl <30 mL/min, using UFH 5,000 units SC twice or three times daily 1, 4, 5
  • If CrCl ≥30 mL/min, the American College of Chest Physicians prefers LMWH (enoxaparin 40 mg SC daily) for VTE prophylaxis in hospitalized dialysis patients 2, 1
