Furosemide Therapy for Dependent Edema

Initial Assessment and Dosing

  • The American College of Cardiology recommends starting furosemide at 20-40 mg orally once daily in the morning, titrating upward every few days until achieving 0.5-1.0 kg daily weight loss, and always combining with an ACE inhibitor or ARB plus a beta-blocker in heart failure patients 1
  • The European Journal of Heart Failure suggests checking serum electrolytes, creatinine, BUN, and volume status before starting furosemide 3
  • The American College of Cardiology advises increasing the dose every 3-5 days until achieving target weight loss of 0.5-1.0 kg daily 1

Mandatory Combination Therapy

  • The American College of Cardiology and the European Journal of Heart Failure recommend combining furosemide with an ACE inhibitor (or ARB) and a beta-blocker to prevent clinical decompensation 1 3

Monitoring Requirements

  • The European Journal of Heart Failure suggests rechecking electrolytes, BUN, creatinine 1-2 weeks after initiation and after each dose increase 3
  • The European Journal of Heart Failure recommends daily weights with patient self-adjustment of dose based on weight changes 3

Diuretic Resistance Management

  • The American College of Cardiology advises increasing frequency to twice-daily dosing if inadequate response despite dose escalation 1
  • Praxis Medical Insights recommends adding a thiazide diuretic (e.g., metolazone) for sequential nephron blockade if inadequate response despite dose escalation 4

Patient Education

  • The American College of Cardiology and the European Journal of Heart Failure instruct patients to weigh daily and adjust dose if weight increases or decreases beyond specified range 1 3
  • Praxis Medical Insights advises patients to take medication in the morning to avoid nocturnal diuresis 4
