Management of Transient Ischemic Attack

Initial Assessment and Treatment

  • The American Heart Association recommends immediate addition of aspirin to existing clopidogrel for 21-30 days of dual antiplatelet therapy in patients with non-cardioembolic transient ischemic attack, followed by return to monotherapy, but only if the patient qualifies as high-risk 1
  • The American Stroke Association suggests that if the transient ischemic attack is cardioembolic, especially atrial fibrillation, clopidogrel should be stopped and switched to anticoagulation, such as apixaban or warfarin, as anticoagulation is superior to antiplatelet therapy for cardioembolic stroke prevention 2

Dual Antiplatelet Therapy Protocol

  • The American College of Cardiology recommends dual antiplatelet therapy for patients with non-cardioembolic transient ischemic attack who meet high-risk criteria, including ABCD2 score >4, minor stroke, or symptom onset within 24 hours 1
  • The dosing regimen for dual antiplatelet therapy includes a loading dose of aspirin 160-325 mg and continuation of clopidogrel 75 mg daily, followed by maintenance therapy with aspirin 81-325 mg daily and clopidogrel 75 mg daily for 21-30 days 1

Evidence Supporting Dual Antiplatelet Therapy

  • The POINT trial demonstrated that dual antiplatelet therapy for 90 days prevented 15 ischemic strokes per 1000 patients treated but caused 5 major hemorrhages per 1000 patients, with a strength of evidence rated as high 1
  • The CHANCE trial showed similar efficacy with 21 days of treatment, with a strength of evidence rated as moderate 1

Long-Term Management

  • The American Heart Association recommends indefinite monotherapy with one of the following: clopidogrel 75 mg daily, aspirin 81-325 mg daily, or aspirin 25 mg + extended-release dipyridamole 200 mg twice daily, for secondary stroke prevention 4, 5
