Management of Fifth Metatarsal Fractures

Classification and Initial Assessment

  • The American College of Radiology recommends proper radiographic evaluation with three standard views for accurate diagnosis of fifth metatarsal fractures, which are among the most common foot fractures 1

Rehabilitation and Follow-up

  • The Annals of the Rheumatic Diseases suggests early introduction of physical training and muscle strengthening followed by long-term balance training and fall prevention for patients with fifth metatarsal fractures 2
  • Prolonged immobilization may lead to stiffness and muscle atrophy, requiring appropriate rehabilitation, as noted by the Annals of the Rheumatic Diseases 2

Evidence-Based Treatment Selection

  • The Journal of the American College of Radiology indicates that CT imaging may be beneficial to evaluate the true extent of osseous injury and guide surgical planning for unstable fractures 1
  • Early fracture fixation provides the most effective pain relief when indicated, according to Anaesthesia 3

Treatment of Non-Displaced 5th Metatarsal Fractures

Diagnostic Approach

  • Radiographic confirmation with three standard views (anteroposterior, lateral, and mortise) is necessary to properly diagnose the fracture, as recommended by the American College of Radiology 4, 5
  • Weight-bearing radiographs provide important information for fractures of uncertain stability, according to the American College of Radiology 5

Monitoring and Follow-Up

  • Regular radiographic assessment to ensure proper healing and alignment is necessary, as recommended by the American College of Radiology 4, 5

Special Considerations

  • For patients with diabetes and neuropathy, special attention should be paid to offloading the foot to prevent complications, as suggested by the American Diabetes Association 6, 7

Diagnosis and Management of 5th Metatarsal Fractures

Special Considerations

  • The Ottawa rules can help determine when imaging is necessary for foot trauma, with specific attention to point tenderness at the base of the fifth metatarsal 8

Initial Management of Base of 5th Metatarsal Fractures

Pain Management During Immobilization

  • Early fracture stabilization through proper immobilization provides the most effective analgesia, as stated by the Anaesthesia guideline society 9

Initial Management of 5th Metatarsal Fractures

Immediate Assessment and Imaging

  • Assess for vascular compromise immediately, checking for signs of poor perfusion such as a blue, purple, or pale foot, and provide emergent intervention if necessary, according to the American Heart Association guidelines 10
  • Check for open wounds that would require wound coverage with clean dressing to reduce contamination risk, following the principles of wound management 10
  • Apply Ottawa ankle rules to determine imaging necessity, which indicate radiographs are required if there is point tenderness at the base of the 5th metatarsal or inability to bear weight for four steps, as recommended by the British Society of Sports Medicine 11

Pain Management

  • Provide multimodal analgesia, starting with scheduled paracetamol (acetaminophen) unless contraindicated, as suggested by the American Society of Anesthesiologists 12
  • Add opioids cautiously, particularly if renal function is unknown, and avoid NSAIDs if renal dysfunction is suspected, following the guidelines of the American Society of Anesthesiologists 12

Immobilization Strategy

  • Splinting reduces pain, prevents further injury, and facilitates safe ambulation, according to the American College of Surgeons 10
  • Immobilize in the position found unless deformity prevents safe transport, following the principles of the American College of Surgeons 10

Special Considerations

  • Do not apply compression wraps too tightly, as this can compromise circulation, according to the American Heart Association guidelines 10
  • Do not place ice directly on skin if using cryotherapy for pain/swelling, as recommended by the American College of Sports Medicine 10

Initial Management of Fifth Metatarsal Fracture

Immobilization and Surgical Referral

  • The American College of Surgeons recommends arranging surgical consultation within 24-48 hours for evaluation of fifth metatarsal fractures, particularly for displaced fractures or Jones fractures 13

Pain Management and Weight-Bearing

  • Early fracture stabilization through proper immobilization provides superior analgesia compared to medications alone, and patients can safely bear weight immediately if pain permits, with progressive weight-bearing not compromising healing and may improve outcomes 13
