Clonazepam Tapering Protocol
Introduction to Tapering
- The American College of Physicians recommends calculating each reduction as a percentage of the current dose, not the original dose, to prevent disproportionately large final reductions that can trigger severe withdrawal 2
- The Mayo Clinic advises against abrupt discontinuation of clonazepam, as this can cause seizures and death 3
Tapering Schedule
- For patients starting at 2.5 mg/day, the tapering schedule may involve reducing the dose to 2.0 mg/day for weeks 1-2, 1.5 mg/day for weeks 3-4, and 1.0 mg/day for weeks 5-6, with further reductions of 0.25 mg per week 1
- The taper rate must be determined by the patient's tolerance, not rigid adherence to schedule, and may need to be adjusted based on the patient's response 2
Managing Withdrawal Symptoms
- Gabapentin can be used as a pharmacological adjunct to help mitigate withdrawal symptoms, starting at 100-300 mg at bedtime or three times daily, and increasing by 100-300 mg every 1-7 days as tolerated 2
- Cognitive-behavioral therapy during taper significantly increases success rates, and patient education about benzodiazepine risks and withdrawal expectations improves outcomes 2
Monitoring and Safety
- Weekly follow-up is recommended during the 7-week taper to monitor for withdrawal symptoms, mood changes, and suicidal ideation 2
- The taper should be paused or slowed if significant withdrawal symptoms emerge, and some patients may need to extend beyond 7 weeks 2
- Patients on clonazepam for more than 1 year may need 10% monthly reductions rather than weekly, and elderly patients require more gradual tapers with smaller dose reductions 2
Special Considerations
- Pregnant patients should not taper without specialist consultation due to risk of spontaneous abortion and premature labor 2
- History of withdrawal seizures requires specialist referral, not primary care management, and patients with unstable psychiatric comorbidities require specialist involvement 2
- Patients should be warned about increased overdose risk if they return to previous doses after tolerance is lost 2
