Treatment of Mycoplasma Pneumonia in Children

Alternative Macrolide Options

  • The IDSA/PIDS guidelines recommend oral clarithromycin at 15 mg/kg/day divided into 2 doses or oral erythromycin at 40 mg/kg/day divided into 4 doses as first-line alternatives for a 3-year-old with Mycoplasma pneumonia who is allergic to azithromycin 1, 2
  • Clarithromycin and erythromycin are well-established and effective against Mycoplasma pneumoniae in children, with clarithromycin possibly preferred due to better tolerability and twice-daily dosing 1, 2

Non-Macrolide Alternatives

  • Doxycycline is not recommended for children under 7 years old due to dental staining concerns, according to guidelines 1, 2
  • Fluoroquinolones, such as levofloxacin and moxifloxacin, are only recommended for older children and adolescents with skeletal maturity 1, 2

Severe Disease Requiring Parenteral Therapy

  • Intravenous erythromycin lactobionate may be used at 20 mg/kg/day divided every 6 hours for severe disease requiring hospitalization or inability to tolerate oral medications 1, 2
  • Transition to oral therapy is recommended when the child is clinically improved 1, 2

Clinical Monitoring

  • The child should be reviewed after 48 hours of treatment, and if not improving or deteriorating, re-evaluation is necessary 3
  • Oxygen saturation should be maintained above 92% if respiratory compromise is present 3
  • Typical treatment duration for macrolide therapy is 5 days 4
