Outpatient Antibiotics for Acute Cholecystitis

  • The American College of Physicians recommends amoxicillin-clavulanate 875 mg/125 mg orally twice daily as the preferred oral agent for non-critically ill, immunocompetent patients with mild community-acquired cholecystitis, providing adequate coverage for common biliary pathogens 1, 2
  • For patients with beta-lactam allergy, the Infectious Diseases Society of America recommends ciprofloxacin 500-750 mg orally twice daily plus metronidazole 500 mg orally three times daily, but notes that increasing fluoroquinolone resistance among E. coli makes this less reliable 3, 1

Critical Exclusions from Outpatient Management

  • The American College of Surgeons recommends immediate hospitalization and IV antibiotics for patients with signs of sepsis or septic shock, immunocompromised state, advanced age with frailty, inability to tolerate oral intake, failed outpatient management, or evidence of complicated cholecystitis 1, 3, 4

Duration of Therapy

  • The Society of Surgical Oncology recommends discontinuing antibiotics within 24 hours post-operatively for uncomplicated cases if cholecystectomy is performed within 24-48 hours 4
  • For delayed cholecystectomy approach, the American College of Physicians recommends continuing antibiotics for 4 days maximum in immunocompetent patients 1

Special Coverage Considerations

  • The Infectious Diseases Society of America notes that enterococcal coverage is not required for community-acquired cholecystitis 3, 1
  • The Society of Surgical Oncology notes that anaerobic coverage beyond what amoxicillin-clavulanate provides is not routinely needed unless the patient has a biliary-enteric anastomosis 3, 1

Common Pitfalls to Avoid

  • The American College of Physicians recommends avoiding ampicillin-sulbactam due to high E. coli resistance rates 3
  • The Infectious Diseases Society of America recommends avoiding fluoroquinolone monotherapy without anaerobic coverage 3
