Factors Affecting PSA Levels

  • The American Urological Association recommends upper reference limits for PSA levels as follows: for men aged 40-49 years, 2.5 ng/mL (Whites), 2.0 ng/mL (Asian-Americans and African-Americans); for men aged 50-59 years, 3.5 ng/mL (Whites), 4.0 ng/mL (African-Americans), 3.0 ng/mL (Asian-Americans); for men aged 60-69 years, 4.5 ng/mL (Whites and African-Americans), 4.0 ng/mL (Asian-Americans); for men aged 70-79 years, 6.5 ng/mL (Whites), 5.5 ng/mL (African-Americans), 5.0 ng/mL (Asian-Americans) 1

5-Alpha Reductase Inhibitor Therapy

  • The National Comprehensive Cancer Network recommends that failure to achieve a 50% PSA decrease on finasteride/dutasteride, or any PSA increase while on these medications, is associated with increased prostate cancer risk 2

Ejaculation and Prostate Manipulation

  • The American Urological Association recommends avoiding PSA testing within 48 hours of ejaculation, as it can cause false-positive elevations 1
  • The European Association of Urology recommends avoiding PSA testing within 3-6 weeks of prostate manipulation, urinary tract infection, or ejaculation 1

Ethnicity and Family History

  • The National Comprehensive Cancer Network recommends that men with first-degree relatives diagnosed with prostate cancer before age 65 have 2.1- to 2.5-fold increased likelihood of diagnosis 4
  • The National Comprehensive Cancer Network recommends that African-American men have 64% higher prostate cancer incidence and 2.3-fold increase in mortality compared to White men 4

PSA Velocity and Kinetics

  • The American Urological Association recommends calculating PSA velocity from at least three consecutive measurements over 18-24 months for reliability 2
  • The American Urological Association recommends that for PSA <4 ng/mL, velocity >0.35 ng/mL per year is suspicious for cancer; for PSA 4-10 ng/mL, velocity >0.75 ng/mL per year is suspicious 2
  • The American Urological Association recommends that PSA increase ≥1.0 ng/mL in any 12-month period warrants immediate evaluation regardless of baseline PSA value 2, 1

Laboratory and Technical Factors

  • The College of American Pathologists recommends using the same laboratory assay for serial measurements, as assays vary by 20-25% 1
  • The College of American Pathologists recommends confirming elevated PSA values after 3-6 weeks, as between 40-44% of initially elevated PSA values normalize on repeat testing without intervention 1

Common Pitfalls to Avoid

  • The European Association of Urology recommends not acting on a single elevated PSA value—verify with a second measurement 5
  • The American Urological Association recommends not ignoring medication history—failure to account for 5-ARI therapy leads to missed cancers 2
