Metoprolol Titration for Heart Rate Control
Titration Protocol
- For a patient currently on metoprolol 25 mg every 6 hours with a target heart rate <120 bpm, increase the dose by 25 mg increments every 6 hours if heart rate remains >120 bpm and the patient shows no signs of hypotension, bradycardia, or heart failure decompensation 1
- The maximum daily dose is 200 mg twice daily for metoprolol tartrate, though the every-6-hour regimen typically transitions to twice-daily dosing after initial stabilization 1
Critical Safety Checks Before Each Dose Increase
- Hold or reduce the dose if systolic blood pressure <100 mmHg with symptoms, heart rate <60 bpm with symptoms, signs of heart failure decompensation, new or worsening bronchospasm, or second or third-degree heart block on ECG 1, 3
Monitoring Parameters During Titration
- Check heart rate, blood pressure, respiratory status, and symptoms of hypoperfusion at each dose administration, with a target heart rate of <120 bpm initially and 60-100 bpm once stabilized 1, 2
Common Pitfalls to Avoid
- Do not increase doses if systolic blood pressure is <120 mmHg in patients with acute coronary syndrome or aortic dissection, and avoid continuing escalation if heart rate drops below 60 bpm 1, 3
- The European Heart Journal recommends avoiding administration to patients with active asthma or decompensated heart failure 3
