Management of Dermoid Cysts in the Neck

Diagnosis and Evaluation

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends a targeted physical examination for patients with neck masses to rule out malignancy, especially if the mass has been present for ≥2 weeks without fluctuation, is fixed to adjacent tissues, firm in consistency, >1.5 cm, or has ulceration of overlying skin 1
  • Imaging studies are essential for diagnosis and surgical planning of dermoid cysts, with MRI preferred as it can show characteristic features including fatty and calciferous content that may be pathognomonic 2
  • Ultrasound typically shows hyperechoic components with acoustic shadowing, hyperechoic lines and dots, and sometimes fluid-fluid levels in dermoid cysts 2, 3

Surgical Management

  • Complete surgical excision is the recommended treatment for dermoid cysts in the neck to prevent recurrence and potential complications, according to the Infectious Diseases Society of America 4, 3
  • The surgical approach for dermoid cysts in the neck should be planned based on the location and extent of the cyst 2

Special Considerations

  • The Infectious Diseases Society of America recommends against gram stain and culture of pus from inflamed epidermoid cysts 4
  • Antibiotics are rarely necessary for inflamed cysts unless there are complicating factors such as multiple lesions, cutaneous gangrene, severely impaired host defenses, extensive surrounding cellulitis, or severe systemic manifestations of infection 4, 3
  • Fine needle aspiration (FNA) should be performed instead of open biopsy for neck masses deemed at increased risk for malignancy when the diagnosis remains uncertain, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 1

Follow-up and Prognosis

  • The American Academy of Otolaryngology-Head and Neck Surgery recommends that evaluation should continue until a definitive diagnosis is obtained and clinicians should not assume the mass is benign for cystic neck masses 1
  • Patients should be advised of criteria that would trigger the need for additional evaluation, and a plan for follow-up should be documented to assess resolution or final diagnosis, as recommended by the American Academy of Otolaryngology-Head and Neck Surgery 1

Potential Complications

  • Untreated dermoid cysts may lead to infection, as reported by the Infectious Diseases Society of America 4
  • Untreated dermoid cysts may lead to rupture causing inflammation, as reported by Praxis Medical Insights 3
  • Untreated dermoid cysts may lead to pressure on adjacent structures, as reported by the American Academy of Otolaryngology-Head and Neck Surgery 1
  • Rarely, malignant transformation can occur in dermoid cysts, more commonly in older patients, as reported by Praxis Medical Insights 2, 3

Palpatory Findings of Dermoid Cysts

Key Clinical Features

  • The American College of Radiology notes that dermoid cysts are three-dimensional and asymmetrical compared to the contralateral side 5
  • Physical examination alone cannot reliably distinguish dermoid cysts from other masses, including malignancies, according to the American College of Radiology 5
  • The American College of Radiology recommends that any highly suspicious mass should undergo biopsy regardless of benign-appearing palpatory findings 5
  • Imaging studies, such as ultrasound, MRI, or CT, are essential for definitive diagnosis, showing characteristic features like fatty content, hair, teeth, and cartilage, as stated by the American College of Radiology 6
  • Red flags requiring further evaluation include firm consistency with indistinct borders, as noted by the American College of Radiology 5
  • Fixation to skin or deep fascia is also a red flag requiring further evaluation, according to the American College of Radiology 5

Palpatory Findings and Diagnostic Approach in Abdominal Dermoid Cysts

Location-Specific Considerations and Diagnostic Algorithm

  • Most ovarian dermoid cysts present as pelvic masses in younger women, with over 75% of ovarian tumors in younger patients being germ cell tumors, and dermoid cysts being the most common, according to the American Society of Clinical Oncology 7
  • MRI is preferred for definitive diagnosis of dermoid cysts, demonstrating pathognomonic features including fatty and calciferous content, hair, teeth, and cartilage, as recommended by the American College of Radiology 7
  • Image-guided core biopsy is preferred over fine needle aspiration for better diagnostic accuracy in suspicious masses, as suggested by the American College of Radiology 8
