Rebound Headache Management

Introduction to Rebound Headache

  • The American Academy of Neurology recommends immediate, complete withdrawal of all overused medications combined with initiation of prophylactic therapy to break a rebound headache, as continuing the offending agents will perpetuate the cycle and prevent any preventive treatment from working 1

Medication Limits

  • The American Headache Society suggests limiting simple analgesics (NSAIDs, acetaminophen) to fewer than 15 days per month to prevent recurrence of rebound headache 1
  • The American Headache Society recommends limiting triptans to fewer than 10 days per month to prevent recurrence of rebound headache 1
  • The American Academy of Neurology advises avoiding entirely medications containing barbiturates, caffeine, butalbital, or opioids, as these carry the highest risk of causing rebound 2
  • The American Headache Society suggests restricting any acute headache medication to no more than 2 days per week to minimize rebound risk 3

Prophylactic Therapy

  • The American Academy of Neurology recommends starting preventive treatment immediately upon withdrawal, with options including topiramate, which has double-blind, placebo-controlled trial evidence specifically in chronic migraine 1
  • The American Academy of Neurology suggests onabotulinumtoxinA as a proven beneficial treatment for chronic migraine when delivered using the Phase III protocol 1
  • The American Headache Society recommends amitriptyline as a drug of second choice for migraine prophylaxis, effective but causing dry mouth, sedation, and weight gain 1

Monitoring and Follow-Up

  • The American Headache Society suggests maintaining a headache diary to track headache days and medication use, facilitating early detection of overuse patterns 1
  • The American Academy of Neurology recommends providing preemptive education on medication overuse headache risk at every visit to prevent recurrence 1
  • The American Headache Society advises against prescribing daily analgesics, as this worsens the problem rather than helping 3
