Hypoglycemia Management in Addison's Disease

Immediate Treatment Protocol

  • Administer 15-20 grams of oral glucose immediately if the patient can safely swallow, according to the American Diabetes Association, as a first-line treatment for conscious patients with hypoglycemia 1, 2, 3
  • Recheck blood glucose after 15 minutes and repeat treatment if levels remain below 70 mg/dL (3.9 mmol/L), as recommended by the American Diabetes Association, to ensure timely and effective management of hypoglycemia 1, 4
  • Once glucose normalizes, provide a meal or snack to prevent recurrence, as suggested by the American Diabetes Association, to maintain stable blood glucose levels 3, 5

Severe Hypoglycemia Treatment

  • Administer 10-20 grams of intravenous 50% dextrose immediately for severe hypoglycemia with altered mental status, as recommended by the American College of Clinical Endocrinologists, as the definitive treatment for Addison's patients 6
  • Recheck blood glucose every 15 minutes until levels exceed 70 mg/dL, as recommended by the American College of Clinical Endocrinologists, to monitor the effectiveness of treatment 6
  • Stop any insulin infusion if present, as recommended by the American College of Clinical Endocrinologists, to prevent further hypoglycemia 6

Target Blood Glucose After Treatment

  • Achieve blood glucose greater than 70 mg/dL (3.9 mmol/L), as recommended by the American College of Clinical Endocrinologists, to ensure adequate glucose levels 6
  • Avoid overcorrection causing iatrogenic hyperglycemia, as recommended by the American College of Clinical Endocrinologists, to prevent complications 6
  • For hospitalized patients, maintain 100-180 mg/dL for noncritically ill patients, as recommended by the American College of Clinical Endocrinologists, to ensure stable blood glucose levels 6
