Ultrasound Characteristics of Neurofibromas
General Ultrasound Characteristics
- Neurofibromas typically appear as hypoechoic, well-defined masses with characteristic features on ultrasound that can help differentiate them from other soft tissue lesions 1, 2
Diagnostic Considerations
- When evaluating neurofibromas, it's essential to assess for potential malignant transformation, particularly in patients with Neurofibromatosis Type 1 who have an 8-13% cumulative risk of developing malignant peripheral nerve sheath tumors (MPNSTs) 3
- Concerning features on ultrasound that may suggest malignant transformation include rapid growth, heterogeneity, irregular margins, and increased vascularity 2
- Biopsy sampling of suspicious neurofibromas should target radiologically concerning but surgically accessible areas, using 14-18G biopsy needles with at least 6 core biopsies if feasible 2
Role in Clinical Management
- Ultrasound serves as an excellent initial imaging modality for suspected neurofibromas due to its non-invasive nature, lack of radiation exposure, and wide availability 4
- For neurofibromas with atypical features on ultrasound, further imaging with MRI may be warranted to better characterize the lesion 1, 5
