Safety of Liquid Nitrogen Cryotherapy on the Lip

Contraindications and Risks

  • The Infectious Diseases Society of America and American Society of Tropical Medicine and Hygiene recommend avoiding cryotherapy on the lips due to high risk of complications, including scarring, tissue damage, and poor cosmetic outcomes 1
  • The lips are a high-risk anatomical site for cryotherapy due to their thin skin, prominent visibility, and unique characteristics, making them susceptible to adverse effects from freezing injury 2, 4
  • Cryotherapy on the lips can result in cosmetic concerns, including hypopigmentation and hyperpigmentation, which can persist for 6-12 months 2, 3

Specific Risks and Complications

  • Severe tissue damage, including vesicle formation, erythema, swelling, and burning pain, can occur immediately after lip cryotherapy 2, 3
  • Permanent scarring, including keloid formation, can occur after lip cryotherapy, particularly on the lips 2, 3
  • Pigmentary changes, including hypopigmentation and hyperpigmentation, can be worse and more persistent in patients with darkly pigmented skin 2, 4
  • Functional impairment, including damage to eating, speaking, and facial expression, can occur due to damage to the sensitive area of the lips 4

Alternative Approaches and Precautions

  • The American Society of Tropical Medicine and Hygiene recommends consultation with dermatology or oral surgery for any lip lesion requiring intervention 1, 4
  • Surgical excision is generally preferred over cryotherapy for better control and cosmetic outcomes on the lips 4
  • No self-treatment is recommended, as the skill of the operator is critical to success and safety 2, 3
  • The Centers for Disease Control and Prevention recommend avoiding the use of cryoprobes in sensitive areas, including the lips, due to additional perforation risk 5, 3
