Treatment Options for Scalp Vitiligo

Initial Approach

  • The British Journal of Dermatology recommends that for patients with skin types I and II, no active treatment might be appropriate, using only camouflage cosmetics and sunscreens if the condition causes minimal cosmetic concern 1, 3
  • Before initiating treatment, the British Journal of Dermatology suggests checking thyroid function due to the high prevalence of autoimmune thyroid disease in vitiligo patients 1, 4
  • Documenting the extent of disease with photographs is recommended to monitor treatment response 5

Topical Treatment Options

  • The British Journal of Dermatology recommends potent or very potent topical corticosteroids for recent-onset vitiligo for a trial period of no more than 2 months, with examples including clobetasol propionate 0.05% or betamethasone valerate 0.1% 1, 2, 6
  • Topical calcineurin inhibitors, such as pimecrolimus or tacrolimus, should be considered as alternatives to topical steroids, with a better safety profile and comparable efficacy to topical corticosteroids in repigmentation 1, 2, 7, 8

Phototherapy Options

  • The British Journal of Dermatology recommends narrowband UVB (NB-UVB) phototherapy for patients who cannot be adequately managed with topical treatments, with a greater efficacy and better safety profile than PUVA 1, 3, 6, 9
  • NB-UVB phototherapy should be used in preference to PUVA, especially for darker skin types, with a safety limit of no more than 200 treatments for skin types I-III 6, 9

Surgical Options

  • The British Journal of Dermatology suggests that surgical treatments should be reserved for cosmetically sensitive sites where there have been no new lesions for at least 12 months, with no Koebner phenomenon present and no extension of existing lesions in the previous 12 months 3
  • Split-skin grafting provides better cosmetic and repigmentation results than minigraft procedures, utilizing surgical facilities that are relatively freely available 3
  • Autologous epidermal suspension is considered an optimal surgical transplantation procedure, but requires special facilities, and is applied to laser-abraded lesions followed by NB-UVB or PUVA therapy 3, 9

Special Considerations for Scalp Vitiligo

  • The British Journal of Dermatology notes that hair-bearing areas may respond differently to treatment than non-hair-bearing skin, and that for extensive scalp vitiligo that cannot be adequately treated, consider camouflage options such as wigs, hats, or scalp micropigmentation 2, 5

Psychological Support

  • The British Journal of Dermatology recommends that psychological interventions should be offered to improve coping mechanisms, and that quality of life impact should be assessed and considered when determining treatment approach 3, 5

Treatment Algorithm

  • For mild to moderate scalp vitiligo, start with potent topical corticosteroid for 2 months, or use topical calcineurin inhibitor instead if concerned about side effects, with consideration of combination therapy with calcipotriene and corticosteroid for enhanced efficacy 1, 2, 9
  • If inadequate response after 2-3 months, switch to alternative topical therapy or combination approach, or consider adding NB-UVB phototherapy, especially for darker skin types 3, 6
  • For extensive or treatment-resistant scalp vitiligo, use NB-UVB phototherapy as primary treatment, or consider surgical options if disease has been stable for at least 12 months 3, 6, 9
