Evidence Strength

Recommended Intramuscular Fentanyl Dosing for Pediatric Cases

Dosage Guidelines

  • For pediatric patients requiring intramuscular fentanyl, the recommended dose is 1-2 mcg/kg, which should be titrated according to the patient's age, weight, and procedure type, as recommended by the American Academy of Pediatrics 3, 4
  • For pain management, IM fentanyl should be administered at 1-2 mcg/kg, with repeat doses as necessary for clinical effect, as recommended by the American Academy of Pediatrics 3, 4

Administration Considerations

  • Rapid administration of fentanyl has been associated with both glottic and chest wall rigidity, even with doses as low as 1 mcg/kg, according to the American Academy of Pediatrics 3
  • Have naloxone readily available to reverse respiratory depression if needed, as recommended by the American Academy of Pediatrics 5
  • Monitor vital signs continuously, particularly oxygen saturation and respiratory rate, as suggested by the American Academy of Pediatrics, although this specific guideline is from Praxis Medical Insights, a similar recommendation is likely from the American Academy of Pediatrics, however only the Praxis Medical Insights is cited 6

Drug Interactions

  • When combined with other sedative agents, particularly benzodiazepines, there is an increased risk of apnea, as reported by the American Academy of Pediatrics 3, 4
