Diagnosis and Management of Diabetes Insipidus
Contraindications and Precautions
- The American Academy of Pediatrics and other medical societies contraindicate the water deprivation test in patients with confirmed nephrogenic diabetes insipidus, especially in infants and young children, due to the significant risk of hypernatremic dehydration and neurological complications 1, 2, 3
- The test is also contraindicated in patients with pre-existing hypernatremia (Na >145 mmol/L) or clinical evidence of dehydration, and should be used with caution in patients with advanced chronic kidney disease (stage ≥G4) 1, 2, 4
Diagnostic Accuracy and Alternative Methods
- The European Society of Endocrinology notes that the water deprivation test has limited diagnostic accuracy compared to modern methods, such as the measurement of copeptin, which can diagnose nephrogenic diabetes insipidus with high precision 3, 5
- The Endocrine Society recommends the use of genetic testing for suspected nephrogenic diabetes insipidus and copeptin measurement for diagnostic differentiation, avoiding water deprivation when possible 3, 5, 6
