Management of Racing Thoughts in Bipolar Disorder

Introduction to Racing Thoughts

  • Racing thoughts are a hallmark symptom of manic and hypomanic episodes in bipolar disorder, characterized by rapid, uncontrollable thought patterns that significantly impair functioning, often occurring alongside euphoria, grandiosity, irritability, increased psychomotor activity, and marked sleep disturbance in patients with bipolar disorder 1, 2

Treatment of Bipolar Disorder

  • The American Academy of Child and Adolescent Psychiatry recommends that lithium, valproate, or atypical antipsychotics (aripiprazole, olanzapine, risperidone, quetiapine, ziprasidone) be used as first-line treatments for acute mania with racing thoughts in patients with bipolar disorder 3
  • Lamotrigine is approved as maintenance therapy for bipolar I disorder, significantly delaying time to intervention for any mood episode (including manic episodes with racing thoughts) in patients with a history of bipolar disorder 3

Maintenance Therapy

  • Maintenance therapy with lamotrigine must continue for 12-24 months minimum, as more than 90% of noncompliant patients relapsed versus 37.5% of compliant patients with bipolar disorder 3

Safety Considerations

  • Lamotrigine must be titrated slowly over 6 weeks to 200 mg/day to minimize risk of serious rash, including Stevens-Johnson syndrome, in all patients 3
  • If lamotrigine was discontinued for more than 5 days, it is recommended to restart with the full titration schedule rather than resuming the previous dose to minimize risk of serious rash in patients with bipolar disorder 3
