Optimal Antiretroviral Management in HIV with Concurrent Antiepileptic Use

Critical Drug Interactions and Management

  • The American Medical Association recommends against using bictegravir with potent CYP3A4 inducers like carbamazepine, as it significantly reduces bictegravir levels and causes antiretroviral treatment failure 1
  • Switching to levetiracetam, which does not induce or inhibit cytochrome P450 enzymes, eliminates the drug interaction concern and is a safe and well-tolerated option 4
  • The guideline-recommended first-line regimen for treatment-naïve adults is bictegravir/emtricitabine/tenofovir alafenamide, with a 92.4% virologic suppression rate at 48 weeks in clinical trials 2, 3, 1

Implementation and Monitoring

  • Consult with neurology to coordinate the antiepileptic switch and ensure seizure control during transition, using a loading dose of 1,500mg and maintenance dosing of 500mg twice daily of levetiracetam 4
  • Initiate bictegravir/emtricitabine/tenofovir alafenamide once levetiracetam is established, and monitor HIV viral load at 4 weeks after starting antiretroviral therapy to confirm virologic response 2, 3
  • Monitor seizure control during and after the antiepileptic transition, and assess for levetiracetam tolerability at 2-week follow-up 4
