Ranitidine Dosing Guidelines for Pediatric Patients

Introduction to Ranitidine Dosing

  • The American Academy of Pediatrics recommends ranitidine dosing at 5-10 mg/kg/day divided into 2-3 doses, with a maximum of 300 mg/day for treatment of GERD or peptic ulcer disease in pediatric patients aged 1 month to 16 years 1
  • Ranitidine is available as peppermint-flavored syrup or effervescent tablets, with the latter having better taste acceptance in children 1

Dosing Considerations

  • For pediatric patients, the recommended dose is 5-10 mg/kg/day divided into 2-3 doses, as supported by the American Academy of Pediatrics guideline for GERD management 1
  • The FDA-approved age range for ranitidine use is 1 month to 16 years, with weight-based dosing as outlined above 1
  • In the context of anaphylaxis, the IM dose is 1 mg/kg per dose, maximum 50 mg, used as second-line therapy in combination with diphenhydramine 2

Limitations and Monitoring

  • Ranitidine develops tachyphylaxis within 6 weeks of continuous use, is less effective than PPIs for healing erosive esophagitis, and requires 2-3 times daily dosing due to its 6-hour acid suppression duration 1
  • Cimetidine has been linked to liver disease and gynecomastia, although this is not specific to ranitidine 1

Ranitidine Dosing in Children

Anaphylaxis Management

  • For anaphylaxis management, the American Academy of Allergy, Asthma, and Immunology recommends a ranitidine IM dose of 1 mg/kg (maximum 50 mg) as second-line therapy combined with diphenhydramine, with a strength of evidence based on clinical trials 3
  • The combination of diphenhydramine plus ranitidine is superior to diphenhydramine alone for anaphylaxis treatment, according to the Journal of Allergy and Clinical Immunology guidelines, with a moderate level of evidence 3
  • For IV administration in anaphylaxis, the recommended protocol is to dilute ranitidine in 20 mL D5W and administer over 5 minutes, as suggested by the Journal of Allergy and Clinical Immunology, with a high level of evidence 3

Ranitidine Use in Anaphylaxis and Critical Care

Dosage for Anaphylaxis (Second‑line Therapy)

  • Intramuscular administration of ranitidine at 1 mg/kg per dose (maximum 50 mg) is recommended as a second‑line adjunct in anaphylaxis, to be used together with diphenhydramine; epinephrine remains the first‑line treatment. 4
  • For adult patients, the recommended intramuscular dose is 50 mg. 4
  • For pediatric patients, the dose range is 12.5–50 mg (approximately 1 mg/kg). 4
  • When given intravenously, ranitidine should be diluted in 20 mL of 5 % dextrose and infused over 5 minutes. 4

Administration Routes in Critical Settings

  • Intravenous ranitidine may be employed in critically ill children (e.g., PICU patients) or in severe anaphylaxis when rapid effect is required. 4
  • Intramuscular ranitidine is indicated for anaphylaxis when intravascular access is not readily available. 4
