Treatment of 3rd Distal Metacarpal Hairline Fracture
Initial Management
- Removable splinting is appropriate for minimally displaced fractures like hairline fractures of the metacarpal, with a duration of immobilization typically lasting 3-4 weeks, and active finger motion exercises should be performed following diagnosis to prevent stiffness 1, 2
- Active finger motion exercises should be performed following diagnosis to prevent stiffness, which is one of the most functionally disabling adverse effects 1, 2
Follow-up Protocol
- Radiographic evaluation should be performed initially to confirm diagnosis and fracture pattern, at approximately 3 weeks post-immobilization to assess healing, and at the time of immobilization removal to confirm adequate healing 1, 2
Indications for Surgical Management
- Surgical fixation would only be indicated if significant displacement (>3mm) is present, dorsal tilt >10° is observed, or intra-articular displacement is present 1, 3
- The American Academy of Orthopaedic Surgeons indicates that dorsal tilt >10° is an indication for surgical management 3
Potential Complications
- Immobilization-related adverse events occur in approximately 14.7% of cases and may include skin irritation and muscle atrophy 1, 2
- Joint stiffness is a potential complication without proper treatment and early motion exercises 1, 2
