Spironolactone Dosing for Primary Aldosteronism

Introduction to Spironolactone Therapy

  • The American College of Cardiology, American Heart Association, and Endocrine Society do not recommend using PASC scores to determine spironolactone dosing for primary aldosteronism patients, instead emphasizing blood pressure control, potassium normalization, and tolerability as treatment guides 1, 2

Special Considerations for Spironolactone Use

  • In patients with eGFR <30 mL/min or significant renal dysfunction, spironolactone should be avoided due to increased risk of hyperkalemia 1
  • For elderly patients with multiple comorbidities, medical therapy with spironolactone is recommended over surgery, using standard dosing with closer monitoring 3

Alternative Therapies for Primary Aldosteronism

  • If spironolactone causes intolerable side effects, eplerenone 50-100 mg daily can be used as an alternative, titrating up to 200-300 mg daily 3, 1
  • Amiloride 5-10 mg once or twice daily can be used if both spironolactone and eplerenone are not tolerated, to control hypokalemia 1

Monitoring and Dose Adjustments

  • Serum potassium and creatinine should be checked within 2-3 days of initiation or dose increases, then again at 7 days, with continued monthly monitoring for the first 3 months 3
  • If potassium rises above 5.5 mEq/L, the dose should be reduced by 50% or switched to alternate-day dosing 3
  • Spironolactone should be discontinued if potassium exceeds 6.0 mEq/L or creatinine rises above 2.5 mg/dL in men or 2.0 mg/dL in women 3

Common Pitfalls to Avoid in Spironolactone Therapy

  • Combining spironolactone with ACE inhibitors or ARBs without close potassium monitoring should be avoided, as hyperkalemia risk increases substantially 1, 2

Expected Outcomes of Spironolactone Therapy

  • Medical therapy with spironolactone achieves blood pressure improvement in virtually 100% of patients when adequate doses are used 3
  • Complete blood pressure normalization occurs in approximately 50% of patients, similar to surgical cure rates 3
  • Early diagnosis and treatment with spironolactone improve cure rates and reduce cardiovascular complications 3

Mineralocorticoid Receptor Antagonist Dosing in Primary Aldosteronism

Standard MRA Dosing Algorithm for Primary Aldosteronism

  • Eplerenone requires twice-daily dosing due to shorter half-life but causes fewer anti-androgenic side effects, such as gynecomastia, erectile dysfunction, and menstrual irregularities, compared to spironolactone, as reported in Circulation Research 4
