Management of STEMI in Patients on Phosphodiesterase-5 Inhibitors
Immediate Management
- The European Society of Cardiology recommends immediate primary percutaneous coronary intervention (PCI) as the definitive reperfusion strategy, while avoiding nitrates due to the critical contraindication with tadalafil, and using morphine for pain control instead 1
- The combination of nitrates and phosphodiesterase-5 inhibitors like tadalafil can cause severe, potentially fatal hypotension, with nitrates being contraindicated for at least 48 hours after the last dose of tadalafil 1
Primary Reperfusion Strategy
- Primary PCI must be performed within 90-120 minutes of first medical contact by an experienced team, with a Class I, Level A recommendation for all STEMI patients with symptoms <12 hours duration 1, 3
- If primary PCI can be performed within 120 minutes, the patient should be transferred directly to the catheterization laboratory, bypassing the emergency department, and proceed immediately to angiography with PCI of the culprit artery 1
Antithrombotic Regimen
- Aspirin has been correctly administered, with a Class I, Level B recommendation, and a potent P2Y12 inhibitor should be added immediately, with prasugrel or ticagrelor preferred over clopidogrel 1, 3
- Unfractionated heparin should be used as a weight-adjusted IV bolus followed by infusion for primary PCI, and enoxaparin IV followed by subcutaneous for fibrinolytic therapy 1, 2
Pain Management
- Morphine sulfate should be used for pain control and anxiety reduction, as nitrates are contraindicated, and it also reduces sympathetic drive and myocardial oxygen demand 1
Statin Therapy
- High-intensity statin therapy is indicated for all STEMI patients, and simvastatin should be continued if the patient is already on this medication 1
Critical Time-Dependent Considerations
- The benefit of reperfusion therapy is highly time-dependent, with the greatest mortality benefit occurring within the first 3 hours of symptom onset, and every minute of delay increasing myocardial necrosis and worsening outcomes 3
Post-Reperfusion Management
- Routine echocardiography should be performed during hospital stay to assess LV and RV function, detect mechanical complications, and exclude LV thrombus after primary PCI 1
- Emergency angiography and PCI should be performed if heart failure/shock develops after fibrinolytic therapy, and angiography and PCI of the infarct-related artery should be performed between 2-24 hours after successful fibrinolysis 1, 2
