Ziprasidone Dosing and Administration
On-Label and Off-Label Use
- For acute agitation in schizophrenia, the American College of Emergency Physicians recommends intramuscular ziprasidone 10-20 mg, with 10 mg showing efficacy but 20 mg demonstrating more significant reduction in agitation scores, and can be repeated every 2-4 hours as needed, up to a maximum of 40 mg/day 1, 2
- The American Academy of Emergency Medicine suggests that onset of action occurs within 15-30 minutes after injection of intramuscular ziprasidone for acute agitation 2, 3
- For emergency setting, 20 mg IM ziprasidone is effective for rapid reduction of acute agitation with onset within 15 minutes, and is effective for agitation related to psychiatric disorders, alcohol intoxication, and substance-induced states 2, 3
Metabolic and Sexual Side Effects
- The American Association of Clinical Endocrinologists notes that ziprasidone is one of the most weight-neutral antipsychotics with fewer metabolic effects, and has a minimal impact on cholesterol, triglycerides, or glycemic control 3, 4
- The American Urological Association reports that ziprasidone has a lower incidence of sexual side effects compared to many other antipsychotics, which may include decreased libido, erectile dysfunction, and delayed ejaculation or anorgasmia 4
Clinical Decision Making
- The American College of Physicians recommends assessing primary condition and symptom profile, ruling out cardiac contraindications, and considering ziprasidone if patient has metabolic concerns or requires rapid control of agitation without excessive sedation 3
