Drug Interactions and Cardiovascular Risk

Introduction to Clopidogrel and PPI Interactions

  • Lansoprazole inhibits the CYP2C19 enzyme, which is necessary for converting clopidogrel to its active metabolite, potentially increasing the risk of cardiovascular events 1
  • The American Heart Association suggests considering alternative acid suppression therapy, such as H2-receptor antagonists (H2RAs), which may be used instead of PPIs, though they provide less GI protection 2

Management of Clopidogrel and PPI Interaction

  • If both medications must be continued, separate administration times by at least 12 hours (e.g., clopidogrel in evening, PPI in morning) to minimize the interaction, as recommended by the American College of Cardiology 2
  • For patients at high risk of cardiovascular events, consider switching lansoprazole to pantoprazole if PPI is needed, or using H2RA instead of PPI if appropriate for GI risk, based on guidelines from the American Heart Association 2

QTc Prolongation and Ondansetron

  • Ondansetron has potential for QTc prolongation, and when combined with other QTc-prolonging medications, there is increased risk of cardiac arrhythmias, according to the National Comprehensive Cancer Network 3
  • Caution is warranted in patients with other risk factors for QTc prolongation when using ondansetron, as advised by the American College of Cardiology 3

Monitoring and Assessment

  • Monitor for signs of reduced antiplatelet effect (cardiovascular symptoms), GI bleeding or symptoms, QTc prolongation symptoms (palpitations, syncope), and anticholinergic side effects from dicyclomine, as recommended by the American Heart Association 2
  • Assess cardiovascular risk for patients at high risk of cardiovascular events, and consider monitoring platelet function if available, based on guidelines from the American College of Cardiology 2
