LMWH Initiation Timing in Ward and ICU Patients

Introduction to LMWH Prophylaxis

  • The American Society of Hematology strongly recommends LMWH over unfractionated heparin (UFH) for prophylaxis in medical patients, with initiation within 24-48 hours of admission once there are no active bleeding contraindications 1
  • LMWH prophylaxis should be initiated within 24 hours of hospital admission for acutely ill medical patients at risk for VTE, with standard prophylactic dosing including enoxaparin 40 mg once daily or dalteparin 5,000 units once daily subcutaneously 1, 2

Ward Patients (Acutely Ill Medical Patients)

  • The American College of Chest Physicians recommends that LMWH is strongly preferred over UFH for prophylaxis in medical patients, with duration continuing for the length of hospital stay or until the patient is fully ambulatory 1, 2
  • Mechanical prophylaxis alone (compression devices) should only be used if pharmacological prophylaxis is contraindicated, and do not extend prophylaxis beyond hospital discharge in standard acutely ill medical patients 1

ICU Patients (Critically Ill Medical Patients)

  • The American Society of Hematology recommends that LMWH is preferred over UFH in critically ill patients, with initiation within 24-48 hours of ICU admission for all critically ill patients without contraindications 1
  • In patients with renal failure (creatinine clearance <30 mL/min), UFH may be preferred over LMWH 2

Contraindications to Assess Before Initiation

  • Absolute contraindications requiring delay include active major bleeding or recent hemorrhage, recent intracranial hemorrhage or stroke, ongoing gastrointestinal bleeding, and severe thrombocytopenia (platelets <50,000/μL) 2, 3
  • Relative contraindications requiring risk-benefit assessment include recent surgery within 48-72 hours (high bleeding risk procedures), epidural catheter in place, and moderate renal impairment (adjust dose or use UFH) 2, 4, 5

Monitoring Requirements

  • Baseline hemoglobin, platelet count, and creatinine should be monitored before initiation, with no routine anti-factor Xa monitoring needed for standard prophylactic doses in ward patients 4, 5
  • In ICU patients, monitor platelet count if LMWH continued beyond 5 days (risk of heparin-induced thrombocytopenia), and consider anti-factor Xa monitoring in obese patients, those with renal dysfunction, or when using non-standard doses 3, 5
