Management of Multiple Benign Appearing Cortical Cysts

Diagnostic Approach

  • Both brain MRI and noncontrast CT scan should be performed for comprehensive evaluation and classification of the cystic lesions, including assessment of location, size, number of cysts, and presence of associated symptoms, according to Clinical Infectious Diseases guidelines 3, 4
  • Careful assessment of cyst characteristics is essential, including location, size, and number of cysts, as recommended by Clinical Infectious Diseases 4

Management Algorithm

  • Conservative management is recommended for asymptomatic benign cortical cysts, with patient counseling and reassurance, and follow-up imaging with MRI at 6-month intervals until stability is confirmed, as suggested by Neuro-Oncology guidelines 1, 2
  • For symptomatic benign cortical cysts presenting with seizures, initiation of antiepileptic drugs is recommended, with choice of drugs guided by local availability, cost, drug interactions, and potential side effects, according to Clinical Infectious Diseases guidelines 5
  • For symptomatic benign cortical cysts presenting with signs of increased intracranial pressure, management should focus on reducing intracranial pressure, with anti-inflammatory therapy such as corticosteroids for cerebral edema, and surgical approach may be necessary for hydrocephalus, as recommended by Clinical Infectious Diseases guidelines 3, 5

Follow-up Recommendations

  • MRI should be repeated at least every 6 months until resolution or confirmed stability of the cystic lesions, and most benign cortical cysts (>80%) do not change in size over time, while a minority may decrease in size or grow modestly, according to Neuro-Oncology and Clinical Infectious Diseases guidelines 1, 2, 5

Important Considerations and Pitfalls

  • It is essential to distinguish between simple benign cysts and potentially infectious causes, such as neurocysticercosis, which would require specific antiparasitic treatment, as recommended by Clinical Infectious Diseases guidelines 3, 4
  • Avoid unnecessary surgical intervention for asymptomatic or minimally symptomatic benign cysts, as this carries risks without clear benefit, according to Neuro-Oncology guidelines 1, 2
