Hypertension Treatment with Lisinopril and Losartan

Efficacy Comparison

  • The European Society of Cardiology recommends both lisinopril and losartan as first-line treatments for hypertension, as they effectively lower blood pressure and reduce cardiovascular events in hypertensive patients 1, 2
  • Lisinopril and losartan demonstrate similar effectiveness in reducing cardiovascular events in hypertensive patients, according to the European Heart Journal 1
  • In elderly patients with left ventricular hypertrophy, losartan showed superior reduction in cardiovascular events, particularly stroke, compared to beta-blockers, as reported in the European Heart Journal 1

Special Patient Populations

Patients with Albuminuria/Kidney Disease

  • The American Diabetes Association recommends either an ACE inhibitor like lisinopril or an ARB like losartan as initial treatment for patients with albuminuria, as they reduce albuminuria and lower blood pressure 2, 3
  • ARBs like losartan are specifically recommended for patients with chronic kidney disease because they reduce albuminuria in addition to blood pressure control, according to the American Heart Association 4

Patients with Diabetes

  • The American Diabetes Association recommends both ACE inhibitors and ARBs for diabetic patients with hypertension, as they are effective in reducing cardiovascular events and improving blood pressure control 2, 5
  • For diabetic patients with coronary artery disease or albuminuria, either an ACE inhibitor or ARB should be part of the treatment regimen, as recommended by the American Diabetes Association 2

Elderly Patients

  • The European Society of Cardiology recommends that both medications are effective in elderly patients, but dose titration should be more gradual due to greater chance of adverse effects 1
  • In patients aged 55 or older, calcium channel blockers or diuretics may be more effective first-line agents than ACE inhibitors like lisinopril, according to the British Medical Journal 6

Monitoring Recommendations

  • The American Diabetes Association recommends monitoring serum creatinine/eGFR and potassium levels at least annually for patients on both medications 2, 3
  • Regular blood pressure checks are necessary to ensure target blood pressure is achieved within 3 months, as recommended by the American Heart Association 7
  • If blood pressure remains uncontrolled on monotherapy, consider adding a calcium channel blocker or thiazide-like diuretic, as recommended by the American Diabetes Association 3, 5
