Treatment Options
- The European Urology guidelines recommend doxycycline 100mg twice a day for 7 days as the first-line treatment for Ureaplasma urealyticum infection 1
- Azithromycin 1.0-1.5g single dose is an alternative treatment option for Ureaplasma urealyticum infection 1
- Levofloxacin 500mg once a day for 7 days is an alternative treatment option for Ureaplasma urealyticum infection 1
- Ofloxacin 200mg twice a day for 7 days is an alternative treatment option for Ureaplasma urealyticum infection 1
Treatment Considerations
- Patients with Ureaplasma urealyticum infection should avoid sexual activity during treatment to prevent reinfection 2, 3
- Sexual partners should be treated simultaneously to prevent reinfection and further transmission 1
- Microbiological cure testing is not usually necessary after treatment completion 3, 4
- If symptoms persist or recur, urine culture and antibiotic sensitivity testing should be performed to guide further treatment 3, 4
Special Considerations
- HIV-infected patients should receive the same treatment as non-HIV infected patients for Ureaplasma urealyticum infection 2, 3
- For persistent or recurrent urethritis, metronidazole 2g single dose plus erythromycin 500mg every 6 hours for 7 days may be considered if the patient has complied with initial treatment and re-exposure has been excluded 2, 3
- Alternative treatment options may be necessary for Ureaplasma urealyticum infections resistant to tetracycline-class antibiotics 3, 4
Prevention and Management
- Increasing fluid intake may help reduce the risk of recurrent urinary tract infections 5
- Patients with recurrent infections should undergo a comprehensive evaluation to rule out anatomical or functional abnormalities 5
- Patients should complete the full treatment course, even if symptoms improve quickly after starting treatment 2, 3
