Hypertension Treatment Initiation Guidelines

Initial Therapy and Agent Selection

  • The American College of Cardiology recommends initiating antihypertensive therapy with 2 first-line agents of different classes for adults with stage 2 hypertension, either as separate agents or in a fixed-dose combination 1, 2
  • For adults with stage 1 hypertension, initiation with a single antihypertensive drug is reasonable, with dosage titration and sequential addition of other agents to achieve the blood pressure target 1, 2
  • The American College of Cardiology suggests first-line options for non-Black patients include ACE inhibitors, ARBs, calcium channel blockers, or thiazide diuretics 1, 2
  • For Black adults with hypertension, initial antihypertensive treatment should include a thiazide diuretic or calcium channel blocker rather than an ACE inhibitor or ARB as monotherapy 1, 2

Combination Therapy and Monitoring

  • Beta-blockers like metoprolol are not preferred as initial monotherapy for uncomplicated hypertension, but have compelling indications in patients with coronary artery disease, heart failure with reduced ejection fraction, or post-myocardial infarction 1, 2
  • Recent evidence suggests beta-blockers may have reduced ability to protect against stroke compared to other antihypertensive classes, though they remain equally effective in reducing coronary events and mortality 3
  • Logical pairings for combination therapy include: (ACE inhibitor or ARB) + (calcium channel blocker or diuretic) 4
  • The combination of ACE inhibitor + calcium channel blocker + thiazide diuretic represents guideline-recommended triple therapy for uncontrolled hypertension, according to Praxis Medical Insights 5
  • Blood pressure should be reassessed within 2-4 weeks after initiating or modifying antihypertensive therapy, with the goal of achieving target blood pressure within 3 months 5
  • When initiating therapy, it is crucial to avoid delaying treatment intensification in patients with stage 2 hypertension and to avoid combining two renin-angiotensin system blockers 1, 2, 5
