Albumin and Furosemide Administration Guidelines

Context-Specific Recommendations

  • The American Association for the Study of Liver Diseases recommends albumin administration together with furosemide after large-volume paracentesis at a dose of 8 g per liter of ascitic fluid removed to prevent post-paracentesis circulatory dysfunction, which is a Level A1 recommendation for LVP greater than 5 liters 1
  • In pediatric nephrotic syndrome, the European Society of Pediatric Nephrology suggests considering furosemide at the end of each albumin infusion in the absence of marked hypovolemia or hyponatremia 2
  • The American College of Chest Physicians states that the combination of albumin and furosemide does not improve mortality, ventilator-free days, or meaningful clinical outcomes in critically ill patients 3, 4, 5
  • In acute heart failure with pulmonary edema, the American College of Emergency Physicians recommends combining furosemide with nitrate therapy, not albumin 6

Practical Administration Guidelines

  • The European Society of Intensive Care Medicine recommends administering furosemide within 2 hours following albumin infusion when indicated 2
  • For cirrhotic paracentesis, the American Association for the Study of Liver Diseases suggests giving albumin during/after the procedure, then resuming diuretics 1
  • Furosemide infusions must be administered over 5-30 minutes to minimize ototoxicity risk, as recommended by the American Society of Health-System Pharmacists 2, 7

Safety Considerations

  • The American College of Clinical Pharmacy advises avoiding the combination of albumin and furosemide in patients with marked hypovolemia, hypotension, severe hyponatremia, or anuria 2, 7
  • High-dose furosemide should not be given for more than 1 week due to ototoxicity risk, as warned by the FDA and recommended by the American Society of Health-System Pharmacists 8, 2, 7
  • It is essential to monitor fluid status, electrolytes, blood pressure, and renal function closely when administering albumin and furosemide, as recommended by the European Society of Intensive Care Medicine 2, 7
