Clozapine Monitoring After Discontinuation: Agranulocytosis and Myocarditis Risk

Dosing and Risk Assessment

  • The American Academy of Child and Adolescent Psychiatry recommends that blood count monitoring should be continued for 4 weeks after discontinuation of Clozapine, regardless of the reason for discontinuation 1, 2
  • The risk of agranulocytosis is approximately 1% in all patients and is potentially fatal 1, 3

Monitoring Protocol After Discontinuation

  • The American Academy of Child and Adolescent Psychiatry guidelines suggest that weekly blood count checks for 4 weeks after discontinuation are obligatory, regardless of the duration of Clozapine treatment or the reason for discontinuation 1, 2
  • If the leukocyte count is less than 2,000/mm³ or ANC is less than 1,000/mm³, immediate stop of Clozapine (if not already done), daily blood count checks, monitoring for signs of infection, and consideration of hematological consultation are recommended 1

Important Clinical Considerations

  • The American Academy of Child and Adolescent Psychiatry recommends that agranulocytosis is reversible if Clozapine is stopped immediately 1
  • It is crucial not to omit the 4-week monitoring period, even if Clozapine is discontinued for reasons other than neutropenia 1, 2
  • Small decreases in leukocyte count should not be ignored, as they can be precursors to agranulocytosis 3
