Histamine-Induced Lightheadedness

Introduction to Histamine Effects

  • Lightheadedness is a recognized symptom of anaphylaxis, which involves massive histamine release, as defined by the medical community, mentioning "dizziness" as a clinical manifestation of histamine release 1

Acute Management of Histamine-Induced Lightheadedness

  • For acute management of significant histamine-induced lightheadedness, the American Academy of Allergy, Asthma, and Immunology recommends positioning the patient appropriately and using pharmacological management, including H1 antihistamines like Diphenhydramine 50 mg IV for acute reactions, and H2 antihistamines for more complete histamine blockade, with Epinephrine and fluid resuscitation for severe reactions with hypotension 1

Prevention and Treatment of Recurrent Histamine-Induced Lightheadedness

  • The American Academy of Allergy, Asthma, and Immunology suggests considering second-generation (non-sedating) antihistamines for prevention of recurrent histamine-induced lightheadedness, and avoiding histamine-rich foods in sensitive individuals, as well as identifying and avoiding histamine triggers 2
  • First-generation antihistamines like diphenhydramine can themselves cause drowsiness and impair performance, which may worsen subjective feelings of lightheadedness, and older adults are more sensitive to both histamine effects and antihistamine side effects, according to the American Geriatrics Society 2
  • Concomitant use of alcohol or other CNS depressants can worsen histamine-induced lightheadedness, as reported by the National Institute on Alcohol Abuse and Alcoholism 2
