Treatment of Granulosa Cell Tumor in Postmenopausal Women
Surgical Management
- The European Society for Medical Oncology recommends performing total abdominal hysterectomy and bilateral salpingo-oophorectomy in postmenopausal women with granulosa cell tumor, as fertility preservation is not a consideration 1, 2
- Complete surgical staging includes infracolic omentectomy, biopsies of diaphragmatic peritoneum, paracolic gutters, pelvic peritoneum, and peritoneal washings, as recommended by the European Society for Medical Oncology 1
- Lymph node dissection should only be performed if nodes appear abnormal on inspection, as retroperitoneal evaluation is not mandatory for sex cord-stromal tumors, according to the European Society for Medical Oncology 1
- The European Society for Medical Oncology suggests performing endometrial curettage to exclude concomitant uterine cancer, which can occur with granulosa cell tumors 1, 2
Adjuvant Therapy Decision Algorithm
- The European Society for Medical Oncology recommends that stage IA granulosa cell tumors require no adjuvant therapy, as they have an excellent prognosis after surgery alone 2, 3
- The National Comprehensive Cancer Network suggests considering adjuvant chemotherapy for stage IC disease with high mitotic index, any stage II-IV disease, or ruptured tumor capsule 2, 4
- The European Society for Medical Oncology recommends the platinum-based BEP regimen (bleomycin, etoposide, cisplatin) as the standard chemotherapy choice when adjuvant treatment is indicated 2
Surveillance Strategy
- The European Society for Medical Oncology recommends lifelong follow-up, as granulosa cell tumors can recur up to 20-37 years after initial diagnosis 5
- The National Comprehensive Cancer Network suggests monitoring inhibin B levels every 2-4 months during the first 2 years, as this is the most sensitive and specific tumor marker 5
- The European Society for Medical Oncology recommends performing CT scans of abdomen, pelvis, and chest every 3-6 months for the first 2 years, and continuing surveillance every 6 months beyond 5 years indefinitely 5
Management of Recurrent Disease
- The European Society for Medical Oncology recommends surgical cytoreduction as the most effective treatment for recurrence, with attempt at complete debulking surgery whenever feasible 5
- The National Comprehensive Cancer Network suggests initiating platinum-based chemotherapy with BEP for at least 3 cycles or carboplatin/paclitaxel for 6 cycles after surgical cytoreduction 5
Critical Pitfalls to Avoid
- The European Society for Medical Oncology recommends not performing routine lymphadenectomy in the absence of suspicious nodes, as this adds morbidity without proven benefit in sex cord-stromal tumors 1
