Carvedilol-Induced Hypotension

Mechanism and Clinical Presentation

  • The vasodilatory side effects of carvedilol are generally seen within 24-48 hours of the first dose or after dose increases, contributing to its hypotensive effects 1
  • Hypotension may be asymptomatic or present with symptoms such as dizziness, lightheadedness, or blurred vision 1

Risk Factors and Management

  • Concomitant use of other hypotensive medications, particularly ACE inhibitors, increases the risk of hypotension 1
  • Volume depletion from diuretic therapy also increases the risk of hypotension 1
  • If hypotension occurs, consider reducing the dose of carvedilol, administering carvedilol and ACE inhibitors at different times during the day, or adjusting diuretic doses in volume-depleted patients 1
  • Elderly patients have decreased baroreceptor response and increased drug sensitivity, making them more susceptible to hypotension 2

Monitoring and Special Considerations

  • Monitor blood pressure and heart rate regularly, especially after initiation or dose increases 2
  • Watch for symptoms of dizziness or lightheadedness, particularly with position changes 1
