Management of Hyperkalemia in Chemotherapy Patients with Solid Tumors
Initial Assessment and Risk Stratification
- The American Society of Clinical Oncology recommends determining if hyperkalemia represents tumor lysis syndrome (TLS), which can occur in solid tumors, though it is rare compared to hematologic malignancies, by checking for the constellation of hyperkalemia, hyperphosphatemia, hypocalcemia, and hyperuricemia, with the presence of at least 2 biochemical abnormalities defining laboratory TLS 1, 4
- Obtain an immediate ECG to assess cardiac risk, looking for peaked T waves, flattened P waves, prolonged PR interval, and widened QRS complex, which indicate urgent treatment regardless of the absolute potassium level, as recommended by the European Society of Medical Oncology 1, 2, 4
Management Algorithm for Hyperkalemia
- For mild hyperkalemia (<6 mmol/L), the National Comprehensive Cancer Network recommends initiating aggressive IV hydration to maintain urine output at least 100 mL/hour, and adding loop diuretics (furosemide 40-80 mg IV) to enhance renal potassium excretion, except in patients with obstructive uropathy or hypovolemia, with a strength of evidence of high 1, 2, 4
- For severe hyperkalemia (≥6 mmol/L or ECG changes), the American Heart Association recommends administering calcium gluconate 10%: 50-100 mg/kg IV over 2-5 minutes to stabilize the myocardial cell membrane, with effects beginning within 1-3 minutes but lasting only 30-60 minutes, and does not lower serum potassium, with a strength of evidence of moderate 1, 2, 3
- The European Society of Cardiology recommends administering combination therapy for maximum effect, including rapid-acting insulin 0.1 units/kg IV plus 25% dextrose 2 mL/kg, with onset within 15-30 minutes, and duration 4-6 hours, with a strength of evidence of high 1, 2, 4
- The American Society of Nephrology recommends considering hemodialysis for severe hyperkalemia, especially with renal failure, oliguria, or refractory cases, as it is the most effective and reliable method for removing potassium from the body, with a strength of evidence of high 1, 2, 3
Special Considerations for TLS in Solid Tumors
- If TLS is confirmed or suspected, the National Comprehensive Cancer Network recommends adding rasburicase for rapid degradation of uric acid, using the same schedule and contraindications as for prophylaxis, with a strength of evidence of moderate 1, 2, 4
- The European Society of Medical Oncology recommends treating hyperphosphatemia if >1.62 mmol/L with aluminum hydroxide 50-100 mg/kg/day divided in 4 doses, with a strength of evidence of low 1, 2
