Hallucinations and Insight in Schizophrenia

Diagnostic Considerations

  • Patients with Charles Bonnet Syndrome (CBS) typically maintain insight that their hallucinations are not real, which is a diagnostic criterion, differing from schizophrenia where lack of insight is a core feature 1, 2
  • True psychotic symptoms must be differentiated from psychotic-like phenomena due to idiosyncratic thinking, trauma exposure, or overactive imagination, and cultural and religious beliefs may be misinterpreted as psychotic symptoms when taken out of context, as noted by the American Academy of Child and Adolescent Psychiatry 3

Treatment and Education

  • Patient and caregiver education about the nature of hallucinations can reduce anxiety and fear, as suggested by medical insights 4
