Crestor and Levofloxacin Interaction

Pharmacokinetic Rationale

  • Levofloxacin is predominantly eliminated renally (80% unchanged) without significant CYP450 involvement, according to Praxis Medical Insights 1, 2

Clinical Evidence

  • The American Heart Association, as published in Circulation, indicates that while no direct interaction studies between rosuvastatin and levofloxacin appear in major cardiovascular guidelines 3, 4
  • The European Association for the Study of the Liver recommends that rosuvastatin should not be used with certain antiviral regimens (e.g., sofosbuvir/velpatasvir/voxilaprevir) due to a 19-fold increase in rosuvastatin exposure via BCRP transporter inhibition 5

Standard Monitoring Recommendations

  • Patients taking levofloxacin should be monitored for fluoroquinolone-related effects, such as tendon pain, neurologic symptoms, or gastrointestinal disturbances, as recommended by Praxis Medical Insights 1, 2

Administration Considerations

  • The administration of levofloxacin should be timed to avoid co-administration with antacids or medications containing divalent cations (calcium, magnesium, aluminum, iron), as these decrease fluoroquinolone absorption, according to Praxis Medical Insights 1, 6

Renal Function Adjustments

  • The dose of levofloxacin should be reduced to 750-1000 mg three times weekly when creatinine clearance is less than 50 mL/min, as recommended by Praxis Medical Insights 1, 2
